Checklist for your first appointment:
New Patient Registration - please click here to go to our secure site and complete our patient registration. If you don't have time to do it now, please come a few minutes before your appointment to complete your paperwork at our office.
Insurance card - if you are using any type of insurance, please remember to bring your card(s). Some vision insurance companies may not provide a card, if they don't your regular medical insurance card is fine or a photo ID of the insured or a parent/guardian.
List of current medications - if you would like to provide them through the online registration, that is fine or if you have a list, we would be happy to copy that for you at our office.
