Checklist for your first appointment:
- Insurance card – if you are using any type of insurance, please remember to bring your card(s). Some vision insurance companies may not provide a card, if they don’t, your regular medical insurance card is fine. We will also need a photo ID of the insured or a parent/guardian.
- Current glasses and/or contact lens boxes – if you are currently wearing contact lenses.
- Current medication information – if you have a list, we would be happy to copy that for you at our office.
